Essential Functions:
Provision of Comprehensive Medication Management
- Provides comprehensive medication management services with patients and providers as established by clinic protocols and pharmacy practice standards, to ensure optimal patient care
- Ensures medications are indicated, effective, safe and convenient. Reviews medication regimens, compiles lists of patient prescribed and over-the-counter medicines, advises patients/caregivers on proper dosage, frequency and side effects.
- Participates in pre-visit planning and coordinates care plan with the patient’s care team and caregivers
- Documents care in electronic health record, consistent with clinic policies and expectations
indirect Patient Care and Quality Improvement (30%)
- Provides comprehensive, general and patient-specific drug information to providers, nurses, and other clinical staff members
- Provides education and training regarding common diseases, medication use and monitoring for treatment and prevention of acute and chronic diseases
- Develops and maintains clinical pharmacy program using population health methodology
- Maintains current base of knowledge through research and meetings to provide information regarding new drugs, clinical trials and testing results to learn more about drug efficacy and side effects
- Assists with pharmacy agreements and contracting, as requested
Qualifications:
Required
- Minnesota pharmacist license, in good standing
- BA/BS degree plus at least eight years of experience, or an advanced degree plus at least six years of experience, or a terminal degree plus at least two years of experience to include a Bachelor’s degree or Doctor of Pharmacy degree from an accredited school of pharmacy.
- Completion of residency with ambulatory care focus (PGY1 or PGY2), or at least 2 years experience as a practicing pharmacist in an outpatient clinical setting
- Ability to travel to multiple work locations, working fully on-site
- Commitment to providing team-based care to diverse populations who have comorbid mental health and medical diagnoses
- Knowledge of and commitment to the principles of diversity, equity and inclusion and an ongoing commitment to working with and across diverse communities
Preferred
- Experience at a Federally qualified Health Center or other ambulatory care setting
- Capacity to work in a highly matrixed work environment. Ability to cope with complexity and ambiguity, and creatively solve problems.
- Experience working with low-income and/or ethnically diverse populations
