The Sawtooth Mountain Clinic is looking for an organized, detail-oriented, personable, and hard-working person to serve as a Health Information/Referrals Specialist. Compile, process, and maintain medical records of Sawtooth Mountain Clinic in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of HRSA. This position is also responsible for the preparation and processing of referrals according to SMC procedures. Strong organizational and communication skills are integral to this role.

Summary of Responsibilities:

Protect the security of medical records, both paper and electronic, to ensure that confidentiality is maintained and patient’s rights are protected
Review records for completeness, accuracy, and compliance with regulations
Support physicians by transcribing medical reports and letters and requesting health information when needed for continuation of care
Maintain an active data base by entering data, such as demographic characteristics, history and extent of disease, diagnostic procedures, or treatment into electronic health record
Retrieve patient medical records from other health care facilities and / or organizations to create a complete and accurate electronic health record
Facilitate patient referral appointments by sending records prior to appointment and ensuring we receive health records from those referrals
Release information to persons or agencies according to regulations
Complete Death Certificates
Scan and reconcile outside reports into the electronic health record in a timely manner
Create and update work flows as needed
Individual projects and responsibilities according to the needs of the Health Information Department
Coordinates the process of referring patients to appropriate clinics and assists patients with scheduling referral appointments
Communicates with patients as to any pre-test/referral instructions and to ensure follow-through with referrals
Updates records to assure all parties have accurate information
Assures receipt of complete orders and information necessary for the delivery of care and services
Employs a process that tracks referrals according to SMC referral procedures, and communicates with the providers referred to, ensures that an appointment is made
Monitors progress of referrals, updating status as progress is made, and following up on patient referral inquires not immediately resolved
All other duties as directed and / or requested by the SMC CEO, Providers, Health Information Department Manager, and Health Care Home Team.

Job Qualifications:

Associates degree in Health Information Technology or similar working experience preferred
Strong attention to detail
Excellent interpersonal and organizational skills
Experience in Electronic Health Records
Knowledge of medical terminology
